THE NETHERLANDS – Dutch Safety Board Investigates Fatal Pipeline Accident at Gunvor Energy

The Dutch Safety Board has opened an investigation into a fatal accident at Gunvor Energy’s tank farm in Rotterdam-Europoort, where one person died and five others were injured during pipeline dismantling work on 13 August 2026.

The incident occurred at approximately 11:30 a.m. while six people, including Gunvor employees and contractors, were disconnecting pipes as part of the decommissioning and dismantling of an industrial installation. Two investigators from the Dutch Safety Board visited the site on the day of the accident to make an initial assessment.

Focus on Pipeline Safety Procedures

The investigation will examine the circumstances surrounding the accident, with particular attention to how the organisations involved managed safety during pipeline opening operations.

Opening pipelines is a common activity across industrial facilities, taking place not only during dismantling projects but also during maintenance shutdowns and repair work. Such operations require clear procedures to establish whether a pipeline can be opened safely, as well as effective communication between employees to ensure that critical safety information is shared accurately.

The Dutch Safety Board aims to identify lessons from the incident that could help other companies undertaking similar activities strengthen their safety procedures. Its investigations are not intended to establish fault or determine legal liability.

Requirements for Seveso Sites

Gunvor Energy is classified as a Seveso company, a designation covering industrial establishments where large quantities of hazardous substances are produced, processed, used or stored.

Under Article 8 of the Dutch Safety Board Decree, the Board is required to investigate major accidents involving companies covered by Seveso regulations. The investigation into the Gunvor Energy accident forms part of this responsibility, with the findings intended to contribute to improved safety practices across the industrial sector.
