Initial Email Enquiry –

I know that many of our VPP members have Emergency Response Teams and some have their own fire trucks.
I would like to share this with them, it concerns a failure of a 5" swivel hose fitting that could have caused serious injury had some one been near the truck when it failed. We documented this as a "Near Miss" and have communicated it to our other Sites (we all have the same type of truck). Please share this with other VPP members. Jimmy Salinas at our DynMcDermott Star site in Freeport, Texas wanted to share some information regarding a near missing involving a 5" hose swivel connection on their fire truck.  Take a look at the picture.  There's a number listed below if you need  more information.  I'll also pass along any more information I receive.

Please share this with the FB and LPCT as appropriate.  When I was in Denver we had 5" Storz fitting fail at the training grounds and moved a propane tank 1.5 feet when it contacted it.

The fitting is a  Harrington brand fitting and the "bearings" were flat teflon disks.  They are in the process of replacing the disks on the reminder of their couplings with a replacement aluminum disk.
