Incident Report Subject: CO2 Deaths Date of first enquiry: Tue 22/04/2003 08:23 Initial enquiry / comment. I thought that one may be interesting to share with other JOIFF members. There are interesting lessons learned : * how safe is the warning/delayed release system under maintenance/upset conditions? * 13 persons in CO2 protected room: only 8 escaped by themselves, 1 died (maximum people allowance, escape routes and door size, training, ...); * the notifier panel installed a year before the accident was FM approved and UL listed but it failed anyway.
