Incident Report Subject: Deluge Nozzle Problem Date of Email reporting Incident: Thu 11/09/2003 16:34 Report Detail: "we have experience a problem with one of our deluge Nozzles supplied by Norfass/Kidde Offshore or GW Sprinkler A/S. The Nozzle is a Constant Flow Type also termed VCS and have been found to contain faulty K ring inserts. I have attached a Safety Alert, which I have asked the Project Safety Specialist to put together, so that you may be able to pass this on to some of our other projects. The other attachment is the Nozzle, which we have experienced the problem with. (Please note however that the diagram shows the K ring insert (item 2) and the Deflector (item 8) in the wrong sequence" "The Deluge Nozzle alert shows a proposed corrective action to fix the situation.
