FATALITY CAUSED BY STEAM FROM BOILER BLOW DOWN VALVE
Safety Alert #2004-22
"A worker had gone out to the location alone to check on the boiler. While inside the boiler shack checking the equipment, the boiler blow down 2" Brass Ball Valve failed releasing steam under pressure with a temperature between 1480C and 1620C."

SWEET GAS WELL BLOWOUT
Safety Alert #2004-23
"During a well work- over, the operator of the well experienced a blowout."

EMPLOYER LIABILITY FOR NEGLIGENT USE OF COMPANY VEHICLES
Businesses in the upstream industry using company-owned vehicles may be interested in this recent decision. The summary put out is: by virtue of the old Highway Traffic Act and the new Traffic Safety Act, an employer is vicariously liable for any use of a vehicle that it allows an employee to drive. The fact that an employee agrees in writing to accept strict limits on the use of the vehicle does not exclude the employer from liability.

CONTROLLING EXPLOSIVE ATMOSPHERES IN VESSELS, TANKS AND PIPING SYSTEMS
There have been a number of fires and explosions resulting in serious injuries or fatalities from improper or lack of purging whenworking with hydrocarbons. This bulletin is a heads up when working with potential explosive atmospheres in vessels, tanks, and piping systems.

IS YOUR COMPANY RIDING THE WAVE?...GET ON BOARD!
The PSC has created a bumper sticker size poster approximately 4 1/2" x 11". This poster is available to all companies who have reviewed the IRP 16 Gap Analysis checklist, completed the declaration form and faxed a copy of this form to the PSC. By completing this form you have indicated that the company has met or exceeds the standard of IRP 16 Basic Safety Awareness Training. A copy of this poster will be mailed out to all registered companies on June 16, 2004.
