"A worker had gone out to the location alone to check on the boiler.
While inside the boiler shack checking the equipment, the boiler blow
down 2" Brass Ball Valve failed releasing steam under pressure
with a temperature between 1480C and 1620C."

EMPLOYER LIABILITY FOR NEGLIGENT USE OF COMPANY VEHICLES
Businesses in the upstream industry using company-owned vehicles may
be interested in this recent decision. The summary put out is: by virtue
of the old Highway Traffic Act and the new Traffic Safety Act, an employer
is vicariously liable for any use of a vehicle that it allows an employee
to drive. The fact that an employee agrees in writing to accept strict
limits on the use of the vehicle does not exclude the employer from
liability.
CONTROLLING EXPLOSIVE ATMOSPHERES IN VESSELS, TANKS AND PIPING SYSTEMS
There have been a number of fires and explosions resulting in serious
injuries or fatalities from improper or lack of purging whenworking
with hydrocarbons. This bulletin is a heads up when working with potential
explosive atmospheres in vessels, tanks, and piping systems.
