CSB Releases Safety Bulletin Emphasizing Dangers of Working on Piping and Equipment That May Contain Flammable or Chemically Reactive Materials

This reminds me of an incident which occurred at our Ammonia IV Plant in the mid Eighties - it was common practice back then to purge lines with Nitrogen as a pass through operation. Believing that the lines were fully purged before welding a worker struck an arc and commenced welding - a subsequent explosion caused him significant chest trauma as a result of the blast - The rescue and recovery to grade was also difficult due to the platform height and restrictive location. Since the investigation showed that pockets of gas can and do remain during this type of purging operation. The current purging arrangement MUST include for at least three pressurisations during the purging to allow for complete removal of product.
