The site was implementing the API B108 Control room
upgrade project, which included relocating two smoke detectors of the
FIKE suppression system. When the contractor removed the first detector,
the FIKE alarm system activated (“ON”), as a notification that something
was wrong with the system. The Utilities department was informed of
the event. In the meantime, the contractor continued working with the
other smoke detector. When the second detector was touched, the FIKE
suppression system was activated. The pressure released by the FM 200
gas caused a pipe failure. The piece of pipe destroyed the acoustical
ceiling and hit a contracted employee, who received minor lacerations
in both arms.
