Subject:    Fatal Accident Report - Following a Death at Whiting Refinery USA

An employee at Whiting Refinery , USA, died when he fell from a ladder. The first information from the investigation has been published and indicates that corrosion was present and the handrail gave way at the bottom of the upright supporting the handrail (photos included in the update). The employee fell as little as three metres (ten feet).

Stairs and handrails are prevalent throughout our corner of the business. Please do not tolerate unsafe stairs or platforms. If a set of stairs, a platform or a handrail look unsafe or if it you aren't comfortable that it would provide fall protection when you need it the most - please report it, put it out of service, get it repaired!!

Please use the one pager in the investigation update, on HSSE notice boards, in safety meetings and toolbox talks, to highlight gain this most dangerous aspect of our business.
