Operational Shift Handover Documentation Request

Initial Email Enquiry –

 

Would you pass this on to the members for me please, As a safety initiative, and given a large percentage of accidents occur at start or near end of shift, I have been looking to improve our production Shift handovers in terms of:
  1. Information between shifts which is particularly important to communicate accurately; i.e. permits-to-workMaintenance in progress, plant out of service, process abnormalities
  2. Improve briefs to personnel who have been absent for longer periods 10 day breaks, holidays, post shutdowns and sickness
At present we have a basic log book system and carry out face to face handovers at the end of each shift, including process operation, maintenance work , temporary mod's, extended isolations.There have been no real problems, but there is an underlying feeling that the improvements can be made by a more formal, structured log book system and procedure to even out inconsistencies across shifts as some logs are better than others.

We have not gone down the computer based log book but this may be an option.

Would be grateful for any examples of a more formal handover log system and any comments on the topic from the membership would be welcome.

Here on Teesside we have just introduced something along the lines which you require. It's early days but believe that it is finding favour of our Crew Commanders who's responsibility it is to complete at shift handover. I will enquire if we have this electronically and circulate to the JOIFF members if available.
