These Process Safety moments developed by BP Refining Process Safety have been developed to help promote discussions on important topics such as Management of Change, Process Hazard Analysis, Shift Handover, etc. These Process Safety moments are each based on a short Flash animation describing a past major accident and slides containing the main lessons.  Here is one relating to the 1966 LPG incident  at the ELF Oil Refinery located Feyzin near Lyon.
